Please have your Florida real estate license information, tax information, a professional headshot, and payment information available. Most applicants should be able to complete the process in approximately 10–15 minutes. Please enter information accurately. Information submitted through this form may be used to prepare your Independent Contractor Agreement, W-9, Florida licensing documents, agent profile, and other onboarding records.
